平成12年度より、高齢夫婦世帯の介護状況について、客観的な評価指標による評価を試みるため、高齢夫婦世帯における被介護者の介護状況を環境・身体介護・心理的介護の3側面から既存の評価指標を用い評価し、それらの介護状況に影響を与えていると考えられる被介護者の介護ニーズ、介護者の介護力との関連を検討することを目的として保健婦による面接調査および保健婦からの聞き取り調査を行った。結果、介護状況に影響を与える因子は、介護者の健康状況、社会資源の認知度、ソーシャルサポート、被介護者の年齢、痴呆の有無であった。社会資源の認知に関しては、現在、社会資源を実際には利用してはいないが、資源の存在を認知しているもののほうが介護状況が良好であった。また、ソーシャルサポートもこれに関連しており、地域との関係が良好で、情報が多くもたらされ、周囲からサポートされることが、介護状況に大きく影響を与えることが伺える。また、介護状態の客観的評価、特に環境的な側面において、面接調査による評価得点が低くても、本人・家族の評価は必ずしも不満足なものではなく、評価にずれが生じている場合もあった。本研究は、介護の客観的な評価を行うことを目的として研究を行ったが、介護保険の認定時に求められる認定調査のような客観的な評価も求められるが、加えて、質的な評価が本人・家族のQOLを確保していくためには重要であることが明らかになった。また、従来から明らかになっているように、介護者の健康状態が被介護者の介護状況に大きく関わっており、被介護者の健康の維持増進とともに、介護者の健康の維持増進を行っていくことが重要である。
