本研究は虐待を予防する観点から,日常の育児を親の側に焦点を当てて検討し,虐待を防止するための親子関係調整プログラムの作成を試みようとするものである。親による子どもの殺害や暴力が虐待とみなされることに疑義を挟む余地はないが,不適切な育児や厳しいしつけと虐待との鑑別は難しい。しかし,虐待を事件となる前に予防するためには,虐待についての共通の定義や概念が必要である。本研究では,文献による虐待の定義を整理するとともに,わが国の虐待に関する人々の認識を調査し,次いで,実際に乳幼児を持つ親を対象に,彼等自身が自分の育児態度をどのように評価しているかについて明らかにしようと試みた。研究着手年度に行なった文献研究では、1997〜2001年に「児童虐待」を扱った国内の看護文献は248編みつかった。これらを虐待発生要因、ケア、予防の見地から分析し、まとめた。また、本年度計画の「虐待に対する認識について」の調査では、720名の対象者の年代により虐待とみなす行為が大きく異なるだけでなく、高齢世代(60歳以上)は父親が自分に対してのしつけが厳しかったと感じていたが、若い世代(30歳未満)は母親も父親と同等にしつけが厳しく、したがって両親とも厳しかったと感じている割合が高いなどの知見を得た。これらの結果を総合すると、思春期の子どもの精神の安定のためには乳幼児期の親子関係が密接に関与するとともに、コミュニケーション技術や対人関係能力の基礎が親子関係によって形成されることが示された。すなわち、虐待予防は乳幼児期の親子の問題としてだけではなく、連続した親と子のライフサイクルの中で、母子保健、精神保健活動等とあらゆる手法を有機的に活用して人間としての行き方や他人との付き合い方をトレーニングし、健全な人間観を持てるように幼児期から取り組むべき問題であるといえる。以上のことから、虐待予防に向けた行動を起こす時期やその対象、方策についての展望をとらえることができた。
