一般線型混合モデルの適用による経年的血圧変動への加齢の影響を解析することで、多重レベル解析の有用性を明らかにする目的の研究を行った。対象集団は愛知県内の2職域からなる40歳以上の男性5,574人で、追跡期間は1997年から2000年である。統計解析には一般線型回帰モデルと一般線型混合モデルを適用し、収縮期血圧と拡張期血圧を結果変数、経過年数を説明変数とし、追跡開始時点でのBody Mass Index、年齢、味つけの好み、飲酒習慣、喫煙習慣、余暇運動量、高血圧家族歴を共変量として扱った。一般線型回帰モデルでは拡張期血圧上昇と加齢との関連性は有意であったが、収縮期血圧との関連性は有意ではなかった。一般線型混合モデルでは収縮期血圧、拡張期血圧とも加齢に伴う有意な上昇が明らかとなった。変量効果モデルの解析結果から、経年的血圧変動には観察開始時の血圧が低いほどその後の上昇勾配が急となる「平均への回帰」の関与が大きいことが推察され、混合モデルではその影響をマクロレベルとして分離できることで、検出力が高まったと考えられる。また成長モデルを一般線型混合モデルとして用いた場合も、結果はほぼ同様であった。本研究結果から、加齢の血圧への影響を縦断的観察により明らかにするには、「平均への回帰」のバイアスを調整するために、一般線型混合モデルによる統計解析が適切であると示された。今後は血清脂質値の経年変化を結果変数としたモデルで、混合モデルによる解析を行う予定である。
