行政の変革として大きな転機となっている市町村合併がどの様に保健師の役割、機能に影響を及ぼしているかを把握する目的で、市町村合併を実施した市町村を対象に調査を行った。172名の保健師から回答を得、その結果を以下に示す。市町村合併によって保健師数が増加したのは9%の市町に過ぎなかったが、保健師数が減少したのは21.5%に上った。保健師数が増加した理由は、保健師の配置場所の増加が主だった。保健師の業務は、67%の市町で業務担当と地区担当の併用を行っていることは合併前後によっても変わらなかった。合併に伴い保健事業が新設・廃止・委託などと変更した市町は71%になった。委託をした業務で最も多かったのは予防接種であるが、中には乳幼児健診や乳幼児訪問指導を委託にした市町もあった。保健師が市町村合併によるメリットとして、(1)視野がひろがった(2)相談できる人が増えた(3)他の市町村の良い部分を共有できたなどがあった。デメリットとしては、(1)事務量の増加(2)地域特性が活かしにくい(3)住民との関係が希薄になったなどがある。社会の変化に伴う保健師の配置に関する文献研究では42文献が把握され、これらは公衆衛生看護活動と保健師の配置、保健業務量と保健師数など量的研究が中心であるが、今後、虐待予防など新たな予防的健康問題に対応した保健師の必要性を考慮して配置を考える日地用があることが示唆された。これらの問題に基づき、保健師活動として新たに求められている分野の一つである児童虐待予防に関するネットワーク事業に焦点をあて、オレゴン州の例を参考に検討した。児童虐待予防ネットワークにおける保健師の役割と機能について、他職種との連携、ボランティアの養成など多くの示唆を得た。
