项目摘要

中高年女性介護終了者の介護体験と地域保健活動に対する認識と行動を明らかにする為に、筋萎縮性側策硬化症(Amyotrophic Lateral Sclerosis,以下ALSと略す)の夫を看取った中高年女性介護者を対象に夫の発病から死別後の体験を半構成面接で聞き取り調査を行った。ALSは、働き盛りの年代での発症が多く、中高年期にある介護者は、子育てや経済的負担等、家庭内の役割変化や社会的な役割遂行の困難等負担が大きい。進行性疾患であるALSの場合、身体機能や呼吸機能の喪失を繰り返しながら死に至るという特別な過程をたどるため、告知や人工呼吸器を装着するか否かの意思決定が家族に求められるという特徴がある。研究対象は、同意が得られたI県・T県在住の40〜60歳代のALSの夫を看取った介護終了者で、看取り後1年から5年を経過した12名とした。データ収集方法は、面接調査とALS遺族会の参加観察を行った。面接調査で主に(1)介護の状況と変化(2)介護終了後の経過(3)介護終了後の人生に介護体験が持つ意味(4)地域保健活動への認識や行動について半構成面接を行った。参加観察データはノートに記述した。面接を録音したものを逐語化し質的記述的分析を行った。分析は研究分担者と行い研究の厳密さを確保した。その結果、ALSの中高年女性介護終了者は、介護プロセスの中で夫と一体化した介護を行い、看取り後も自己喪失と深い悲嘆を経験していた。しかし援助者からの見守りや同じ体験を語れる仲間との出会いの中で新たな自己を取り戻し、他者への支援に関心を向けるよう変化したことが明らかになった。他者を支援しようとするパワーを持った介護者は、自分が受けた支援の恩返をしたい、自分の体験を生かしたいと認識しており、そういった変化は、介護中から介護終了後まで援助者から継続的に支援を受けていることが重要な意味を持つことが明らかになった。今回、充分分析できなかった看取り時の状況と援助者の支援のあり方について今後、明らかにしていく必要がある。
