项目摘要

本研究の目的は,訪問看護師の看護実践上の課題である医療モデルから生活モデルへのシフトに着目した自己効力尺度を開発することであった.平成16・17年度の研究成果に基づいて109の質問項目を選定し,訪問看護師57名を対象とした予備調査を実施した.予備調査結果に基づく項目分析により「QOLにつながる訪問看護実践効力尺度」原案87項目が選定された.次に本調査において,訪問看護師263名を対象とし,信頼性を,Cronbach' αの信頼性係数と折半法で求め,妥当性は,構成概念妥当性,並存的妥当性,内容妥当性,で求めた.すべての分析にはSPSS13.OJ for Windowsを用いた.なお,今回実施した調査は,聖隷クリストファー大学の倫理委員会の承認を得て実施している.(以下に詳細を述べる)1)信頼性:内的整合性は,Cronbach's α(0.97),折半法(0.92)である.2)妥当性:(1)最尤法プロマックス回転による因子分析の結果,「利用者・家族とのパートナーシップ形成能力」,「在宅ケアチームアプローチ能力」,「自己の気づきを高める能力」,「セルフケアにつながる成果設定能力」,「シームレスケア提供能力」の5因子から構成する「QOLにつながる訪問看護実践効力尺度」(43項目)が示された.(2)並存的妥当性は,「QOLにつながる訪問看護実践効力尺度」と特性的自己効力感尺度との相関(r=0.59)によって確認した.(3)内容妥当性は,共同研究者4名と心理学専門家1名によって確認した.今後の課題には,病院から在宅へ移行した訪問看護師を対象とした継続教育プログラムに活用できる簡易尺度として,有用性を高めるために,尺度の信頼性と妥当性をさらに検討することが挙げられる.

The purpose of this study is to investigate the nursing practice of nurses and to develop their own performance standards. The research results of Heisei 16 - 17 were selected based on 109 questions, and 57 nurses were interviewed. Based on the investigation results, 87 items were selected from the original project "QOL". 263 nurses were interviewed in this survey. The results showed that the reliability coefficient of Cronbach 'α was the highest, and the validity of the concept, the validity of the coexistence, and the validity of the content. The analysis of SPSS13.OJ for Windows was performed. The investigation was carried out today and the approval of the Ethics Committee of the University of San Francisco was carried out. 1) Reliability: Internal integration: Cronbach's α (0.97), halved method (0.92); 2) Appropriateness: (1) Results of factor analysis of the most specific method: "user family's ability to form a system,""in-house ability,""self-improvement ability,""in-house ability to set results,""in-house ability to provide services," 5-factor composition: "QOL performance scale"(43 items). (2)The appropriateness of coexistence is confirmed by the correlation between the QOL and the self-perceived strength scale of the characteristics (r = 0.59). (3)The content is appropriate, 4 co-investigators and 1 psychology expert are confirmed. The future problem is that hospitals are moving from home to home, visiting nurses, responding to education, using simple standards, improving the usefulness, reliability, and appropriateness of standards.
