项目摘要

高齢者の昼間睡眠の意義とその効果測定法の開発を目的に施設内高齢者を含む様々なステージの高齢者に対し、昼間睡眠ポリグラフ(PSG: Polysomnography)検査を実施した。本年度は施設内有病高齢者9名(平均年齢72.4±7.4歳)、健常成人7名(平均年齢36.7±7.4歳)に対し、昼間睡眠として最も眠気を及ぼしやすい13時から17時の時間帯にPSG検査を行った。記録は短時間仮眠記録とし、記録時間は自然覚醒までとした。全就床時間(TIB: Time in Bet)は高齢者群が55.4±13.8分、健康成人群が71.4±21.4分で、同様に全睡眠時間(TST:Total Sleep Time)は高齢者群が24.9±12.6分、健常成人群が37.1±14.5分で高齢者群が低値を示した。一方、入眠潜時(SL: Sleep Latency)は高齢者群が13.5±8.0分、健常成人群が20.3±11.3分でStage2に至るまでの時間は短時間であった。これらのことから、高齢者の短時間仮眠は短時間で入眠に至るが睡眠時間は短く、覚醒しやすい特性があることが示された。昼間睡眠の構造特徴としては、高齢者群においてStage2の出現時間が12.4±6.6分で、健常成人群(22.1±12.8分)に比し短く、TIBに占める覚醒時間出現率が高値であった。全体では年齢とStage2の出現時間が負の相関関係(r=-0.472、p=0.064)を示し、加齢とともにStage2の出現時間が短縮する傾向が認められた。アテネ不眠尺度とTSTとの間にr=0.452(p=0.079)の正の相関関係が認められた。さらに、高齢者群では入眠に至るまでの時間とStage2の総出現時間が負の相関関係(r=-0.772、p=0.011)を示し、入眠に至るまでの潜時が長い高齢者ほどStage2の出現が短縮された。高齢者の昼間睡眠特性はStage2の出現時間に影響を受けていることが示唆された。

The meaning of daytime sleep in patients with high risk of hypertension is to determine the meaning of daytime sleep. the purpose of this study is to determine whether people with high levels of daytime sleep should have high levels of daytime sleep, and daytime sleep cycles (PSG: Polysomnography) should be used.
