平成18年度までの研究により、寝たきりを過度に恐れる気持ち「寝たきり恐怖」は高齢者が明確に自覚しているものではなく、調査票等による把握は困難であることがわかった。そして寝たきり恐怖の自覚の有無にかかわらず虚弱高齢者に普遍的なアプローチすること、高齢者自身が日常生活の中で転倒の危険要因に自ら気づき、自ら適切な対応をとれるようになることが有効と示唆された。そこで本年度の本研究の目的は、中山間地域の地域高齢者を対象に、日常生活(環境)の転倒の危険要因に気づく力を高めるためのプログラム「転倒の危険予知訓練」を実施し、その効果を明らかにすることとした。本研究に同意した中山間地域在住の認知症を認めない特定高齢者60名を対象とし、小冊子学習群、KYT学習群に同数ずつ振分け、20分間学習してもらった。小冊子学習群は、研究者が作成した住環境整備の対策が書かれた小冊子を配布し、それを熟読してもらった。KYT学習群は、研究者が作成したKYTシートを用い、KYT基礎4ラウンド法を用いて実施した両教材とも文献を参考に同じ内容を含むよう筆者が作成した。転倒危険箇所を描いた住環境のイラストについて、対象者が指摘した危険箇所の個数を成績とし、学習実施直前および直後、1ヵ月後に判定を実施した。その結果、両群とも学習直後で有意な成績向上を示し、1ヵ月後も維持されていたが、2群間に有意差はなかった。向上した者は、転倒不安、活動の減少の訴えがあった。学習後に住環境整備を実施した者は、学習の継続を希望した。以上より、住環境整備の実施には意欲等の心理要因、活動等の身体要因が関与していることが示唆された。
