项目摘要

口蓋形態が構音獲得に及ぼす影響を解明する目的で口蓋裂形成術術後患者の4歳時(歯科矯正治療が開始される前)での上顎模型を用いて形態分析を行い形態の違いと異常構音発現との関連性について検討した.今回は,片側性唇顎口蓋裂に用いる哺乳床(Hotz床:手術前歯槽形態改善の目的でも用いられる)の及ぼす効果についても検討した.1)模型計測:前年と同様に以下の手順で行った.(1)三次元形状計測器(サーフレーサー)を用い三次元形態をスキャンする.(2)スキャン画像をパーソナルコンピューターに取り込み三次元形態解析ソフトで三次元画像に再構築する.(3)構築した三次元模型画像上に計測基準点を設定し,基準平面を決定する.(4)模型画像上に計測点を定め,距離や面積,体積などを測定する.2)口蓋形態の変化に伴う構音状態の分析得られた片側性唇顎口蓋裂(UCLP)患者の模型をHotz床の有無別に正常構音症例,異常構音発現症例に分類し,名々に現れた形態的特徴について検討した.なお,異常構音は口蓋化構音発現症例のみを対象とした.3)結果Hotz(+)群とHotz(-)群の異常構音発現状態と形態とを分析し比較したところ,以下の結果が得られた.(1)Hotz床(+)群は(-)群に比べ,口蓋化構音発現頻度が低かった.(2)前年度に得られた口蓋化構音発現群に特徴的な口蓋形態(a.口蓋後方部の表面積が有意に狭いb.矢状断面において口蓋前方部の膨隆が有意に小さいc.非対称性が強い)はHotz床(-)群に強く認められた.(3)以上より,Hotz床の使用は口蓋形態を改善し,口蓋化構音発現の減少に効果をもたらすことが伺われた.以上について,国内雑誌に投稿,掲載された.また,本研究課題の総説について外国語雑誌に投稿中である.

The purpose of this study was to investigate the relationship between morphology and abnormal articulation at age 4 after cranioplasty (before orthodontic treatment). This paper reports the results of the study on nursing beds for lateral cleft lip and palate (Hotz bed: the purpose of improving the shape of dental grooves before surgery). (1)Three-dimensional shape measuring device (2)The three-dimensional image is reconstructed by analyzing the three-dimensional shape of the image. (3)Set the reference point and determine the reference plane on the three-dimensional model. (4)2) The analysis of flap morphology and associated articulation states was performed on the model of patients with UCLP. 3) The results of Hotz(+) group and Hotz(-) group are compared. (1)Hotz bed (+) group is lower than (-) group. (2)In the previous year, the flap morphology (a. the surface area of the posterior part of the flap is intentionally narrow, b. the expansion of the anterior part of the flap is intentionally small, c. the asymmetry is strong) is the Hotz bed (-) group. (3)As mentioned above, the use of Hotz bed has improved the shape of the cover, and the appearance of the cover structure has been reduced. The above is the first time that the Chinese government has submitted articles to the Chinese government. This research topic has been discussed in detail.
