项目摘要

本研究では、法医学関連の情報を公衆衛生学的に分析し、実態に即した解決策を検討する新学問領域「法医公衆衛生学」の構築を試みた。これまでの3年間の経過を通じ、法医学関連情報として「法医剖検事例」の分析および「死体検案例」の分析双方を有機的に結びつけ分析することで、jこれまで実態が不明であった「虐待」「孤独死」「事故」「自殺」などの実態および関連要因を分析できることが明らかになった。「法医剖検事例」においては、事情聴取があるため、社会的背景など、フォームをあらかじめ作成し聴取する依頼をすることにより詳細な背景データが入手できた。しかし、火災は剖検率が高いが、病死は少ないなど、サンプリングバイアスがあり、剖検例の発生割合をそのまま公衆衛生学的な事例の発生割合と解釈することはできない。一方、「死体検案例」は、状況が不明な死のかなりの部分をカバーしていると考えられ、情報には限界があるが、全体の数および内訳分布などを把握するには適していると考えられた。具体的分析成果としては、まず、「剖検例からみた高齢者死亡の実態と背景要因」として、高齢者の剖検事例を対象として、家族形態によって状況が異なり、家族がいても事故などの異状死体が多いこと、高齢者の火災死亡には、コンロや灯明など特有の要因があり環境整備で予防しうるものがあることなどを、原著論文として発表した(厚生の指標56,1-7,2009)。2年目から開始したフォームによる剖検事例の背景情報の把握も蓄積され、最終年度に高齢者の死亡を分析し学会発表(日本公衆衛生学会2008)を、また、小児虐待の実態把握も行った。「死体検案例」については、まずは前述の死因別の剖検率を算出し、とくに火災では高いなど死因により異なることを、また死亡から発見までの期間による分類では、死因、発見場所、家族状況によって異なること、一方、自殺は家族同居者に多いことなどが明らかになった。また本研究班会議を開き、これらの分析結果を地方行政関係者と共有し、このように各種事例の属性別のパターンを明らかにすることで、「発生予防」および「早期発見」に分けた具体的政策も立案しやすいことが明らかになった。
