项目摘要

胃亜全摘術を予定されたASA-Iの男性14名硬膜外リドカイン投与群(1%リドカイン10ml)…………………L群(7例)硬膜外フェンタニール投与群(フェンタニール10μg・kg^<-1>)…F群(7例)測定項目:心拍数、血圧、心拍出量・血中エピネフリン、ノルエピネフリンコルチゾール、ヒスタミン〈結果〉両群間で年齢、身長、体重、術中出血量、尿量、輸液量に有意差を認めなかった。血中エピネフリンは、両群ともに麻酔導入前後で平均32ng/lから44ng/lと上昇したが有意差を認めなかった。L群は、硬膜外注入後15分、30分値で注入前に比較し有意な減少を認めた。また注入後90分、120分値で投与前に比較し有意な上昇を認めた。F群は、硬膜外投与後45分(執刀15分)値で69ng/lと注入前に比較し有意な上昇を認めたが、その後安定し120分値まで注入前に比較し有意な上昇を認めなかった。血中ノルエピネフリンは、両群とも麻酔導入前後で平均162ng/lから320ng/lと有意に上昇した。硬膜外注入後L群、F群ともに注入後60分値まで注入前と比較し有意差を認めなかったが、L群では注入後90分、120分値で注入前と比較し有意な上昇を認めた。平均血圧、脈拍は、L群で注入後15分、30分値で有意に低下した。血中コーチゾール、ヒスタミンとも麻酔導入前後、注入前後で両群とも有意差はなかった。〈考案〉L群は、血圧、脈拍を有意に減少させ、血中エピネフリン濃度も有意に低下させた。F群は、L群に比較し循環抑制も軽度であった。血中エピネフリン濃度は、L群程には低下、抑制を認めなかったもののその抑制作用は、L群よりも長時間作用した。また血中ヒスタミン濃度は、両群ともに注入前後を通して有意な変動を認めずフェンタニール投与が起因となるヒスタミン遊離による循環動態の変動に影響を与えなかったと思われた。〈結語〉硬膜外大量フェンタニール麻酔は、1%リドカイン1回注入法と比較して、循環動態を緩徐に抑制し、かつ長時間手術侵襲による血中カテコラミンの上昇を抑制するため、循環動態の変動が少なく良好な麻酔維持が可能であることが示唆された。
