In order to perform the transseptal left heart catheterization more safely preventing severe complications such as cardiac tamponade, we have devised several thinner catheter systems for the procedure. They were the transseptal catheter system of 3 French-size, the system of 4 French-size, the system of 5 French-size and the system of 6 French-size. All of them except the 3 French-sized system consisted of the each sized transseptal catheter, the new transseptal needle of 0.032 inch size and the new transseptal guide-wire of 0.018 inch size. Transseptal left atrial catheterizasion could be done with the 3 French-sized system easily and safely, but the 3 French-sized catheter could not be introduced into the left ventricle through the mitral vulve. The newly devised 4 French-sized transseptal system, made from Mallincklodt high flow catheter, could be introduced into the left ventricle through the atrial-septum and the mitral vulve safely. But it's catheteral tip rubbed the ventricular wall and caused the occurrence of the PVCs. The balloon-tipped 5 French-sized system could not be passed into the left ventricle throgh the mitral valve. The newly designed 6 French-sized system, had the balloon at their catheteral tips, could be introduced into the left ventricle transseptlly through the mitral vulve. The balloon formation made the introduction of the catheter system into the left ventricle easier, and made the frequency of the PVCs lower. We believe that the transseptal catheterization using our newly devised 6 French-sized system, made of a balloon catheter, is safer and more useful than the conventional transseptal approach.
为了更安全地进行经间隔左心导管术,防止心脏填塞等严重并发症,我们设计了几种更薄的导管系统。经鼻中隔导管系统法式3例,法式4例,法式5例,法式6例。除3个法国大小的系统包括每个尺寸的跨间隔导管、0.032英寸的新跨间隔针和0.018英寸的新的跨间隔导丝外,其余均为法式系统。3法式系统可方便、安全地进行经间隔左房插管,但3法式导管不能通过二尖瓣置入左心室。由Mallincklot高流量导管制成的4法式跨间隔系统,可通过房间隔和二尖瓣安全地进入左心室。但导管尖端与室壁发生摩擦,导致室上性心动过速的发生。球头5法式大小的系统不能通过二尖瓣进入左心室。新设计的6个法国大小的系统,导管尖端有球囊,可以通过二尖瓣瓣口经间隔进入左心室。球囊的形成使导管系统更容易进入左心室,并降低了PVCs的频率。我们认为,使用我们最新设计的法国尺寸的球囊导管系统进行经间隔插管,比传统的经间隔插管更安全、更有效。
