1.これまでに94例の生体部分肝移植術が施行されており、それらすべての症例でドナー肝に対する術前のMRI検査および術中の超音波検査が施行された。内訳は男性40名、女性54名、平均年令32才である。2.移植肝としては1例で右葉を使用した以外は左葉もしくは外側区を使用しており、すなわち左肝静脈と中肝静脈の解剖が大きな意味を持つ。外側区域を移植肝として使う場合は左肝静脈の本数や、左肝静脈へ流入してくる内側区域枝(V4)の有無、その大きさが問題となる。左葉を移植肝として使う場合は、左肝静脈と中肝静脈との共通幹の有無、中肝静脈へ流入する前上区域枝V8の有無とその大きさが問題となる。3.(1)左肝静脈はその分枝形態に変異が多く、左肝静脈が2本ある症例が16%、3本ある症例が3%あり、また左肝静脈が1本の症例のうちで根部1cm以内で3本以上に分枝するのが20%、2本に分枝するのが13%存在した。鎌状間膜周囲の静脈の多くは左肝静脈へ流入するものの、内側区前上部からの静脈(V4)が下大静脈あるいは中肝静脈根部へ流入するのが35%において認められた。(2)中肝静脈と左肝静脈とが共通幹を有する例が71%に、それぞれが独立して下大静脈へ流入する例が29%に認められた。径3mm以上の前上区域枝(V8)が35%に認められ、そのほとんどが中肝静脈へ、一部は下大静脈へ流入した。4.これらの変異を術前に知りおくことにより円滑な肝移植術に貢献でき、今後の移植術式の決定において大きな意味を持つ。

1. In 94 cases of liver transplantation, preoperative MRI examination and intraoperative ultrasound examination were performed. There are 40 males and 54 females in the inner circle, with an average age of 32 years. 2. In one case, the right lobe was used except for the left lobe, and the lateral region was used except for the left hepatic vein. In the lateral region, the number of left hepatic veins, the inflow of left hepatic veins, the presence of medial regional branches (V4), and the presence of large hepatic veins are all problems. (1)Left hepatic vein branches varied in morphology. Left hepatic vein branches were found in 16% of cases and in 3% of cases. Most of the veins in the middle hepatic vein flow into the left hepatic vein, and 35% of the veins in the anterior upper part of the medial region flow into the inferior great vein. (2)71% of the cases with common hepatic vein and 29% of the cases with common hepatic vein. 35% of the anterior superior regional branches (V8) with a diameter of more than 3mm were found to flow into the middle hepatic vein and one into the inferior great vein. 4.
