本研究の目的は、高齢者における耳垢の蓄積と関連因子及び耳ケアとの関係を明らかにし、耳のケアの意義を考察することである。本年度は昨年度の結果を踏まえ、耳のケアに関する実態調査を行い、さらに耳のケアを実施し、ケア介入後追跡調査を行った。・実態調査A介護老人保健施設の入所者のうち、本人と家族の同意を得た65歳以上の高齢者42名に対し、「耳の診察」「聴力(気導・骨導)検査」「耳のケア」「コミュニケーション」等調査した。年齢84.3±6.7歳、NMスケール33.3±12.4点、BI66.7±26.5点。耳孔が両側もしくは片側完全閉鎖した者(以後完全閉鎖群)は13名31.0%、平均聴力(気導4分法)は右46.7±15.5dB左47.4±18.7dB、完全閉鎖群は完全閉鎖なし群より聴力低下(右:49.9±20.8dB>45.2±12.6dB左:47.8±20.6dB>47.2±18.1dB)がみられた。また、「聴力支障度」と聴力との間に正の相関関係をみとめ、コミュニケーションと聴覚機能との関係が明らかとなった。現在、詳細データについて統計処理中。・介入後の追跡調査上記対象のうち、医師が看護職による耳垢除去を許可及び本人が同意した22名に耳のケア(耳介〜軟骨部外耳道の耳垢除去)を実施し、介入後の状態を追跡調査中。介入直後の聴力は、右42.3±16.6dB左41.1±12.5dB。気導-骨導差は介入前と比べ小さく(右6.2±6.4dB>0.6±4.3dB、左8.6±9.8dB>3.9±6.4dB)、「聞こえがよくなった」13名59.1%「気持ちいい」17名77.3%と感じていた。これらの結果より、耳のケアの介入による主観的・客観的聴覚機能の改善が示唆された。現在、1ヶ月間毎週「耳の診察」及びケア後1ヶ月の「聴力検査」等追跡調査中。以上の結果を踏まえ、耳のケアの意義について考察をすすめてゆく。

The purpose of this study was to investigate the correlation factors between earwax accumulation and earwax loss in patients with high earwax. After the intervention, the force was 42.3±16.6dB on the right and 41.1±12.5dB on the left. Before intervention, the difference between air conduction and bone conduction was smaller than that before intervention (right 6.2±6.4dB>0.6±4.3dB, left 8.6±9.8dB>3.9±6.4dB), and 13 cases were 59.1% and 17 cases were 77.3% respectively. The result of this is that the main function of the host and the guest function are improved. Currently,"ear examination" every week for one month and "strength examination" for one month after failure are under follow-up investigation. The above results show that there is a great deal of research on the significance of the study.
