项目摘要

(1)過去3年間にみられたわが国小児科領域における血球貧食症候群(HPS)のアンケート集計結果をまとめ日本小児血液学会雑誌6:560、1992に掲載することが出来た。別冊をアンケート協力施設に配布した。(2)上記HPS集計結果ならびにLangerhans'cell histiocytosis(LCH)についてのわが国小児科領域での集計結果から組織球増殖性疾患の実態をまとめ、平成4年10月Histiocyte Societyの第8回総会(ドイツ、ハノーバー市)において発表した。(3)HPS自験症例の活動期血清を用いて各種サイトカインを測定し、hypercytokinemiaが予後予測因子となるかどうかについてまとめ投稿中であった論文はAm Jpediatr Hematol/Oncolに掲載された。(4)3歳女児のHPSで、重篤な臨床経過をとり、抗腫瘍剤の投与にも抵抗して6ヶ月で不幸の転帰をとった症例について、脾摘組織の検討から本例にはベースにCD56+のnatural killer(NK)‐lymphomaがあったことを見いだした。また、脾組織にEBVのgenomeを証明し、EBV含有細胞のmonoclonalな増殖を証明しえた。現在、さらにin situ hybridization法をもちいて、EBVのlymphomacellへの局在を検討している。本症例は従来から確定診断の困難な小児HPSの本態について大きな手がかりを与えるものと考えている。(5)研究班員、班友ならびに協力者の諸施設より、新鮮HPS症例の発症時、全血、凍結血清の供与を受け、末梢血リンパ球サブセットの動態ならびに血清サイトカインを測定し、HPS症例の亜型分類が可能か否か引続き検討している。同時にEBV関与についても調ベ、EBV陽性例についてはclonalityをSouthern法により検討している。

(1)In the past three years, the results of the study of hematological deficiency syndrome (HPS) in the field of primary school children were published in Journal of the Japanese Society of Hematology 6:560 and 1992. EBV genome in spleen tissue, EBV monoclonal proliferation Now, in situ hybridization, EBV and lymphomacell are under discussion. This case is difficult to diagnose because of the nature of HPS. (5)The research team, the team members, and the collaborators set up various settings, the onset of fresh HPS cases, the supply and reception of whole blood and frozen serum, the determination of the dynamics of peripheral blood and serum, and the classification of HPS cases. At the same time, EBV related and EBV positive cases were detected by Southern method.
