项目摘要

多数めまい症例のデータ解析を行うことは、めまい疾患の特徴を正確に把握して診断精度の向上を計り、症例に適合した治療法を選択するために重要な方法である。本年度、私達はめまいの発症誘因、発症時状況、随伴症状などのカルテ情報を時系列データとしてデータベース化する情報処理システムを開発した。このシステムではこのような自覚症状の変化と平衡能検査の経時推移、治療内容、症状経過を同時に時系列データとして取扱い経時的解析が可能なように設計した。1987より1991年まで当科を受診しためまい・平衡障害症例について上記のデータベースを作製した。登録項目は病名、自覚症状としてめまいと関連症状の発症時、その程度の経時変化、他覚所見として自発頭位眼振の速度、頻度、温度性眼振検査などの実験的眼振検査、聴力検査所見、施行された治療内容である。登録されたデータは一次的分析で病名に対応した症状の変化と検査所見の解析を行った後に、自覚症と他覚所見の変化の相関性、治療による自覚・他覚所見の変化などを相関分析、判別関数を使用した分析で解析を行った。本年度はとくにメニエール病と類似疾患、前庭神経炎・顔面神経麻痺・聴神経腫瘍などの後迷路性前庭障害を中心に解析を施行した。今回の研究ではメニエール病の重症化に関する問題、富山県におけるメニエール病の地域特性の問題、顔面神経麻痺症例における後迷路性障害、突発性難聴と後迷路性前庭障害との関係について別表のような研究を発表行った。また、メニエール病発症と気候の影響などについての研究を継続しており、作製したデータベースについてさらに各種疾患の治療効果の解析が進行中である。
