対象と方法:1984年1月-1993年12月の10年間に東京大学付属病院耳鼻咽喉科を受診した聴神経腫瘍新鮮例102例106側を前記群(1984-1988年)36側と後期群(1989-1993年)70側に分け臨床像、症状、検査所見等を解析した。結果:(1)受診動態;初発から診断迄の平均の期間は前記群49カ月に対し後期群35カ月と短縮していた。しかし、後期群では初診から診断までの期間は短縮していたが、初発から受診までの期間は前記群と差がなかった。(2)腫瘍最大径;前記群25±11mm、後期群18±8mmと後期群では最大腫瘍径は有意に小型化していた。(3)自覚症状:蝸牛症状が94%と最多で、次いで前庭症状が54%であった。他の神経症状は前記群の47%に対してこうきぐんでは21%と半減していた。(4)神経学的所見;三叉神経、小脳症状などの神経学的所見は、前記群に比して後期群では半数以下に激減していた。(5)聴覚検査所見;前記群と後期群に差はなかった。(6)平衡機能検査所見;温度眼振検査には前記・後期群間に差はなかった。しかし、後期群では中枢障害を示す例は少ない。(7)画像検査;前記群ではMRI未検査例も多い。MRIの診断率は99%と高率であった。結論:(1)MRIの発達・普及に伴い近年の聴神経腫瘍は早期に発見されるため小型化している。(2)腫瘍の小型化に伴い症状・検査所見は軽度化している。(3)医療機関受診から診断確定までの機関は短縮しているが、症状の初発から医療期間を受診するまでの期間は変わらない。患者への早期受診の啓蒙が重要と思われる。

Methods: During the ten years from January 1984 to December 1993, 102 cases of neuroma were newly diagnosed in the Department of Otorhinolaryngology, University of Tokyo Hospital, 106 cases were recorded in the former group (1984-1988), 36 cases were recorded in the latter group (1989-1993), and clinical images, symptoms, and findings were analyzed. Results:(1) The average duration from initial diagnosis to final diagnosis was 49 months, and the average duration from initial diagnosis to final diagnosis was 35 months The period of initial diagnosis is shortened, and the period of initial diagnosis is shortened. (2)The largest diameter of the tumor was 25±11mm in the former group and 18±8mm in the latter group. (3)Self-symptom: snail symptom is 94% and vestibular symptom is 54%. His neurological symptoms decreased by 47% and 21% respectively. (4)Neurological findings; trigeminal neurosis, microcephaly, and neurologic findings are reduced by less than half compared with the previous group. (5)The first group of records and the second group of records are different from each other. (6)Balance function inspection found; temperature eye vibration inspection before the record, after the group between the difference In the late stage, the central damage is shown in the example. (7)Image examination; most of the cases not examined by MRI were recorded in the previous group. MRI diagnostic rate is 99%. Conclusion:(1)MRI development and popularization are accompanied by early development and miniaturization of neuroma in recent years. (2)The miniaturization of the tumor is accompanied by symptoms. The examination shows that the tumor has changed gradually. (3)Medical institution diagnosis confirmed, institution shortened, symptom initial, medical treatment period, period of treatment Early diagnosis of patients is important for enlightenment.
