本研究は重症心身障害のある医療的ケアが必要な子どもを養育する世帯について、生活上の課題を明らかにすることにより、改善の方策の検討に資することを目的としている。子に重い障害があり、在宅で介護と医療のケアを行うような場合、親は就業が容易でなく、経済的社会的に苦境に陥るリスクが高くなる。重い障害がある医療的ケア児を育てるひとり親世帯及びふたり親世帯にインタビュー調査を行い、生活の遂行に際して不足するサービスや支援の内容について整理し、課題の改善や解消に必要な内容について抽出する。ふたり親世帯については、ひとり親世帯の結果を検証する比較群として収集する。初年度はインタビュー調査で使用する調査票の作成に際し、医療的ケアが必要な子どもを育てる親を対象とした質的調査に関する先行研究を収集し、検討した。母親へのインタビューに基づく在宅療養における困難事象の構造研究及び母親自身の健康管理を通じた生活上の課題研究を踏まえ、質問項目案を作成した。並行して調査対象者を機縁募集したが、関係機関が医療関連であったためCOVID-19の感染状況により募集が難航した。2023年３月より募集を開始することができたため、2023年４月からインタビューガイドに基づく半構造化インタビュー調査を行っている。今後、インタビュー調査を継続しテキストに基づく質的分析を行う予定であるが、現在までに実施したインタビューにおいては、主に教育・福祉の地域資源の格差に起因する親の就労の困難性や経済面の負担、福祉・医療サービスの利用のしづらさによる生活の質の低下などの課題が明らかとなっている。

This study aims to explore the medical needs, parenting, and life issues of severe psychosomatic disorders. Children are in serious trouble, home care, medical care, travel, employment, and social hardship. In the case of serious medical problems, the content of medical care support should be sorted out and extracted from the necessary content for improvement of medical care. The results of the study were compared and analyzed. In the first year, we will collect and discuss the preliminary research related to the preparation of the survey ticket for medical treatment and education, as well as the quality of the survey. Research on the structure of difficult problems in home care and research on the problem of mother's own health management In parallel, investigators are seeking information from related authorities on the status of COVID-19 infection. March, 2023: Recruitment begins, April, 2023: Basic semi-structured research begins. In the future, we will carry out basic qualitative analysis on the basis of research and development. We will carry out research and development on the causes of regional resource gaps in education and well-being. We will focus on the problems of low quality of life in the context of the burden on the environment, well-being and utilization of medical services.
