项目摘要

本研究は、医学的緊急性が低く帰宅可能と判断される軽症の救急車利用者を対象として、個人の属性情報と救急車利用に関する情報に基づき、類似の特徴をもつ小集団に分類し、小集団ごとの健康・心理・社会的ニーズを特定することを目的としている。具体的には、①マーケティング分野のセグメンテーション理論を活用し、消防本部から入手した救急要請記録データを用いてクラスタリング手法により軽症の救急車利用者を小集団（セグメント）に類型化する、②救急医療に関わる専門職を対象に半構造化インタビューを実施し、軽症の救急車利用者の各セグメントの健康・心理・社会的ニーズの抽出を行う。2022年度は、主に上記①の研究を実施した。2016～2020年の5年間の広島県東広島市消防局から提供された救急要請・搬送記録データを入手し、本研究の解析に必要なデータの抽出とクリーニングの処理を行った。東広島市消防局で5年間に救急搬送された成人約14,000名のうち、軽症の救急車利用者である成人5,982名を分析対象者として、年齢、性別、病歴、救急搬送時の症状などの個人属性情報と、発生場所、事故種別、同乗者、救急隊員が判断する緊急度などの救急要請に関する項目を用いて、確率的潜在意味解析（ Probabilistic latent semantic analysis: PLSA）により成人の軽症救急車利用者を小集団に類型化した。また、同年度末に東広島市消防局と合同会議を実施した。結果のフィードバックをいただくとともに、成人の軽症救急車利用者の小集団ごとの健康・心理・社会的ニーズを特定するためのインタビュー調査の実施に向けての調整を行った。
