项目摘要

医療従事者のモラルディストレス(倫理的苦悩)軽減効果が期待されているMoral Case Deliberation (以下MCD)は、倫理的に悩ましい事例の話し合いにおいて、話し合い参加者同士の対話を行い、相互理解を促すオランダ発祥の方法論である。日本では、アメリカ発祥の医療倫理の四原則に則り、Jonsenの四分割表などを用いて話し合いを行われることが多い。この方法は情報の整理には向いているものの、関係当事者の価値や信念に焦点を当て、対話することは主目的とされていない。MCDはモラルディストレス軽減効果が見込まれるものの、先行研究から臨床現場でのMCD実施には①話し合いの実施時間確保の難しさ、②MCDファシリテータ養成の必要性の課題が見出されていることが明らかとなった。これらの課題を考慮して、MCDを臨床において実践可能となるよう、ファシリテータ養成プログラムの内容を検討し素案の基盤となる方法論を決定することができた。また、参考としてオランダアムステルダム大学主導で行われているMCDファシリテータ養成プログラムの国際オンラインコースでは、約50時間におよぶ養成プログラムが実施されており、座学と実践が含まれている内容を考慮し、日本の臨床現場での実施可能性を検討しつつ、本研究のファシリテータ養成プログラムの内容を検討していく事となった。今後は、ファシリテータ養成プログラム(案)の内容を精査・検討し、研究実施フィールドの調整と実施可能性の検討を行っていく。
