糖尿病性腎症は慢性透析患者の原疾患第一位である。糖尿病を有する透析患者は心血管合併症を有し、死亡リスクが高いことが明らかとなっている。さらに、透析患者の高齢化に伴い透析導入時の身体活動量の低下は生命予後にも影響するため、糖尿病性腎症の発症早期からの身体活動量への介入が重要である。本研究は、座位行動時間の短縮と低強度身体活動量の増加に向けた支援を構築することを目的とする。本研究では、①糖尿病性腎症患者の身体活動量の実態調査を低強度身体活動尺度（SLPAQ-10）と3軸加速度計を用いて横断調査を行う②低活動患者への座位行動時間短縮および低強度身体活動量増加のための介入計画を立案し、アクションスタディの手法を用いて身体活動に関する介入を行い、臨床データの変化を評価する。2022年度は、糖尿病性腎症患者の活動量を評価するにあたり、低活動量患者の影響因子について国内外の関連文献を広くレビューした。そのレビューをふまえて、行動変容技法が必要とされる糖尿病患者のセルフマネジメントへの介入を検討するために、自己効力感を測定することとした。多く開発されている自己効力感尺度のうち、身体活動・運動に特化した尺度（the Exercise Self-Efficacy Scale,ESES）を用いて、糖尿病性腎症患者の身体活動量と自己効力感について横断調査を実施することとした。また、対象を糖尿病透析予防指導外来での介入中の患者に限らず、糖尿病性腎症で通院中の患者を対象とした。倫理審査承認後に調査を開始する。
