项目摘要

急速な高齢化により、加齢による心身機能の低下「フレイル」の有症率は急増し、その発症予防の確立は人生100年時代である本邦において緊急課題となっている。我々はこれまでに、高齢者を対象とした『見え方』（視覚）に影響を与えうる角膜形状に関する大規模調査を進めてきた(Yoshida et al. Ophthalmic Epidemiol 2021)。また、『見え方』を通しフレイルを予防することを目的に解析を行い、良い『見え方』が社会参加を有意に促進することを見出した(Yoshida et al. Soc Sci Med 2020)。われわれは、認知機能低下をきたした患者に対して, 白内障手術前後での認知機能の変化を検討した。白内障手術を施行した75歳以上の患者に対して, 認知機能検査（ミニメンタルステート検査(MMSE)）を使用して, 術前と術後(3ヶ月後)の認知機能を測定した。軽度認知症群において, 白内障手術後, MMSEスコアが有意に上昇した。また軽度認知症群は認知症群と比較して, 有意に認知機能改善を認めた。本研究は, 認知機能障害が進行していない患者への白内障手術は, 認知機能を改善させる可能性があることを示唆した(現在本研究の成果は論文にし投稿中である)。また、加齢に伴う視覚障害（アイフレイル）が心身機能全般に関連するフレイル（身体的フレイル、心理的・認知的フレイル、社会的フレイル、オーラルフレイル）に影響を及ぼし、それらを加速的に悪化させることによって、健康寿命を短縮させるという仮説を検証することを目的に、視野検査を施行した緑内障患者（300症例）を、視野障害の重症度により分類し、うつ病、認知症、転倒に関わる病歴との関連性を後ろ向き研究にて検証している。本研究の成果より、アイフレイルの観点から新たなフレイル予防法を構築し、超高齢社会のニーズに応えたい。
