项目摘要

われわれはこれまで幼児期の自閉症圏障害を関係性の障害の視点に立って母子治療を試みてきた。その中で自閉症児と養育者との間のコミュニケーション障害に対してまずは情動的水準での改善を試みることによって、母子間でのコミュニケーションは短期間で著明な改善を示し、状況に適切な発語が多くの例で認められた。本研究では、自閉症圏障害への治療実践を通して、情動的コミュニケーションの進展過程と母子双方の音声活動がどのように変容していくか、相互の関連性を検討することを試みた。具体的にその手始めとして、母子相互間の情動的コミュニケーションの進展過程を良好群と不良群に分けて、そこでの母親のvocal markerの生起数を比較検討した。その結果、良好群ではvocal markerの出現頻度は全体的に増加傾向を示していた。ただその増加の度合いは症例による個人差が非常に大きいことが明らかとなった。また不良群でへはvocal markerの生起数の変化よりもpseudo-vocal markerの生起数の多さが治療介入前後を通して一貫して多い傾向にあった。今回の結果はいまだ予備的段階の研究ではあったが、vocal markerが情動的コミュニケーションの進展過程において重要な役割を担っていることが推測された。さらに情動的コミュニケーションが進展しがたい症例において一見vocal markerと類似しているが実は質的に大きな違いがあるpseudo-vocal marker、すなわち子どもの動きとは連動せず、養育者主導の音声活動であるため音声そのものが子どもの意図とはずれているという特徴をもった音声活動が主に出現していることを確認された。最後にvocal markerが象徴的コミュニケーションへの進展過程において果たす役割を知覚様態の特徴との関連性でもって考察した。

This is the first time that a child has been diagnosed with autism. In the middle of the autism spectrum, parents and children have to try to improve the level of emotional response, and mothers and children have to improve the level of emotional response in the short term.
