项目摘要

背景:電子カルテでは,病院の医療系職員が以前よりも容易にアクセスできる潜在的可能性があり,その結果として患者情報が知ってしまう人の数が増えることが危惧される.我々はこれを「情報浸透」とよび,情報漏洩とは区別して取り扱う.今後の医療情報システムは情報浸潤を制御できる必要があると考え,その視点で情報システムのあり方,アクセス管理のあり方を検討した.方法:東大病院の情報システムのアクセスログ記録機能を改造し、入院患者の入院期間中のアクセスをすべて記録するようにし、このアクセスログを解析した。結果:1日あたりの1入院患者にアクセスする異なる医療スタッフの数が5%以下の頻度となるのは、指導医によるアクセスが10人以上、研修医で5人以上、看護師で10人以上の場合であった。また異なる診療科の所属者がアクセスする頻度は1日あたりアクセス診療科数が5を超えるケースがまれにあるだけであった。このことから、これらの条件を満たす場合になんらかのチェック機構を設ける事が妥当である。一方で今回の調査から、2〜5診療科でのグループ診療が行なわれ電子化された診療情報が医療スタッフ間で共用されている実態も明確になった。処方指示や検査指示といった直接診療行為の指示に伴って発生するアクセス(指示アクセス)、指示アクセスの関連結果を参照するといったアクセス(従属型アクセス)、およびこれらに依存しないアクセス(独立型参照アクセス)の3区分に分類し、独立型参照アクセスのうち臨床的にアクセス説明が必要と考えられる頻度は全体の0.76%であった。考察:分析結果から内省的考察により、アクセス時に警告する基準案として、アクセス日時と診療イベントとの日数の距離の条件、アクセス者と患者の診療上の関係条件の2つの点からチェックを行うことを提案し、通常と異なるアクセスと判定した場合にはアクセス理由を入力させるシステムを提案した。

Background: The number of people in the medical department of a hospital is increasing because of the potential for information on patients and the risks associated with it.
