2017.12～2019.3にICUに入室した患者のうち、入室後48時間以内にNGALを測定している患者を対象とし（すでに腎代替療法を行っている患者を除く）、対象の患者が尿中NGAL測定後1週間以内にAKIの診断基準を（KDIGO診断基準）満たしたかを調べ、AKI群と非AKI群に分類し測定値を比較したが、2群間の尿中NGAL測定値に有意差はなかった。入室48時間以内での測定では、すでにAKIに進展しているケースが多かったのではないかと考えている。今後の課題は、全例での入室時の測定を徹底し、もう一度AKI予測が可能であるかを検討したい。そのうえでhANPによる治療介入の有用性を検討していきたい。ICU入室した腎代替療法を行っていない敗血症患者を対象とし、全例ICU入室時に尿中NGAL測定を行い１３０ng/ml以上の患者は早期介入プロトコルとし、直ちにnANPもしくは生理食塩水の投与を行う予定であるが、症例数がまだ不十分である。

From December 2017 to March 2019, NGAL was measured in ICU patients within 48 hours after admission, and the diagnostic criteria for AKI (KDIGO diagnostic criteria) were adjusted within 1 week after NGAL was measured in urine. The classification of AKI group and non-AKI group was compared. The difference in NGAL measurement in urine between groups 2 was intentional. AKI progress is measured within 48 hours of entering the room The future of the project is to determine the time of entry thoroughly and to discuss the possibility of AKI prediction. The usefulness of therapeutic interventions is discussed in detail. In ICU, renal replacement therapy was performed in patients with hematosis. In all cases, NGAL in urine was measured at 130ng/ml or more. In patients with early intervention, the number of cases was not very high.
