甲状腺眼症の症状の１つである眼球突出は、眼窩脂肪と外眼筋の腫大により眼窩の体積が増加するために生じ、QOLを低下させる。そのため眼球突出の改善には眼窩脂肪の体積を減少させる治療法が必要であるが、その治療法は現在、眼窩脂肪の除去を行う観血的手術のみである。一方、緑内障点眼薬であるBimatoprost点眼に眼窩脂肪を減少させる作用があることが近年明らかになった。本研究の目的は、Bimatoprost点眼が増加した眼窩脂肪の体積を減少させ、甲状腺眼症の眼球突出を改善させる作用があるか検討することである。本研究の治療法は、従来の観血的治療法と比較して非侵襲かつ簡便であり、どの施設でも治療可能なため、甲状腺眼症の新しい有用な治療法になると期待される。本研究はこれまでに、眼球突出に対し1年間Bimatprost点眼が可能な甲状腺眼症を対象として研究した。結果、12か月間Bimatoprost点眼を行った9例9眼における眼球突出度、眼窩脂肪・全外眼筋の体積は、いずれも治療前後で有意差を認めなかった。以上より、甲状腺眼症の眼球突出に対するビマトプロスト点眼は有意差のある効果を認めなかった。また本研究に関連する研究として、緑内障に対してBimatprost点眼を片眼にのみ点眼している患者も同様の方法で解析した。結果、Bimatoprost点眼による眼窩脂肪の減少と眼球陥凹をMRI画像で定量的に明らかにできた。Bimatoprost点眼による眼球陥凹は、眼窩脂肪の減少のため生じると考えられた。
