项目摘要

＜研究の目的＞本研究の目的は、心疾患の手術を受けた患者への看護師による退院指導の調査を行い、パソコン用のアプリケーションソフトを用いた心疾患手術後患者・家族への退院指導のモデル（以下心疾患退院指導アプリケーション）を作成することである。研究は、第1段階では、看護師が実施している現状の退院指導と退院後の患者・家族を調査し、実際の指導とその結果を明らかにする。第2段階では、第1段階で得られた調査結果を分析して心疾患退院指導アプリケーションを作成する。その後、心疾患退院指導アプリケーションを導入し、有効性を検証する。＜研究の概要＞1）平成30年12月～平成31年3月:全国の心臓血管外科専門医認定修練施設の基幹施設350施設（心臓血管外科手術が100例/年以上）に勤務する心疾患手術を受けた患者の退院指導経験のある看護師を対象に実態調査をした。①患者の認識の確認や行動のケア、②喫煙に関する指導、③運動に関する指導、④運動に関する指導、⑤心理・社会的ケア、⑥体重管理の指導、⑦血圧管理の指導、⑧糖尿病がある患者の指導、⑨抗凝固薬内服患者の指導、⑩今後の支援の情報提供、⑪術後の創部の管理や一般的な指導の11項目に分別し項目別の実施度と重要度を調査した。その結果、実施度が高い項目は、栄養、血圧管理、糖尿病管理、抗凝固薬管理、今後の支援、術後の創部管理や一般的な指導であった。重要度は全項目において高かった。2）平成31年4月～令和元年6月：全てのデータをまとめて、6月に学会発表を行った。3）令和元年7月～9月：自由記述の内容分析を実施し、現在も継続中である。
