项目摘要

本研究の目的は介護老人福祉施設における感染症対策として、ポジティブデビアンスアプローチによる介入を行い、その効果を検討することである。ポジティブデビアンスアプローチ（以下、PDアプローチ）は、「良い」結果を生んでいる「当たり前ではない見過ごされた解決策や埋もれた知恵（PD行）」を引き出しそれらを集団の中で実践し共有するプロセスである。第1研究では、施設の感染対策上の課題を明確にするため、グループフォーカスインタビューを実施し、おむつ交換時の手指衛生ができていない事が課題としてあげられた。第二研究ではおむつ交換時の手指衛生ができない理由の探索とおむつ交換時の手指衛生に関するPD行動を探索するためグループディスカッションと直接観察法を用いて職員のおむつ交換時の手指衛生の状況を観察した。観察では、おむつ交換時にディスポーサブルタオルで手指衛生を実施している職員がいることを発見した。令和4年度は、第3研究として現在ディスポーサブルタオルでの手指衛生行動が感染対策として有効であるか、おむつ交換業務に従事する施設職員を対象に手指衛生行動別におむつ交換後の手指汚染状況をパームスタンプ法で調査し、ディスポーサブルタオルでの手の拭き取りをしている職員の手に大腸菌が最も少ないという結果が得られた。しかし、ディスポーサブルタオルで手を拭いている職員が1人しかいなかった事や、手袋交換が清潔にできていたこともあり、ディスポーサブルタオルで手を拭くことで手に付着した大腸菌を少なく出来ると証明するには、追加で菌がタオルに移行するのか検証が必要である。本研究は令和5年度の日本看護科学学会にて報告予定である。

The purpose of this study is to investigate the effects of intervention on infectious diseases in elderly care settings. The following is a summary of the results of the "Good" test.
