本研究は、人口高齢化の進展が著しくその現象が地域社会の諸構造に大きな影響を与えていると考えられる中山間地域農村に注目し、高齢者家族の形態(規模や世代構成など)、家族員の就労状況、高齢者の家族における位置や役割といった家族状況を踏まえながら、当該家族と家族を支持する地域の公的・私的システムとの関連を解明することを狙いとしたものである。主たる対象地として岩手県東磐井群藤沢町を選定し、ほぼ通年にわたって断続的に同町を訪れインフォーマント調査を中心に研究を進めた。同町は、長年「医療過疎」に悩みながら町ぐるみの運動を展開して1993年町民病院を開設し、同年ボラントピアセンターという福祉公社方式のボランティア組織を設立するなど、保健・医療・福祉の連帯を独自のスタイルで実践している地域である。本研究では特に、こうした行政組織を中心とした諸機構をひとつ包括的な家族支援システムとして見ることを通じて、その役割と機能、またその波及効果を明らかにしようと試みた。同町でもやはり家族規模の縮小、高齢者世帯・独居高齢者の増加などの傾向は著しく、農家においては経営の弱体化、一般的な世帯における介護福祉機能の低下が深刻化している。同町ではこうした状況に対する危機感から地域的な支援体制づくりに取り組み、現在、病院、特別養護老人ホーム、老人保健施設など保健・医療・福祉にかかわる組織や施設の殆どが町営あるいはそれに近い形を取っているが、このことが保健・医療・福祉の連携・一体化を形式的に実質的にも保障している。町民病院長がこれらのサ-ヴィスを行政機構上統括する位置にあることで医療関連スタッフと施設ならびに在宅ケア・スタッフの緊密な連絡調整が可能になり、これが総合的で充実したサ-ヴィスにつながっている。こうした体制は地域住民の生活支援に大きく貢献しており、過疎化や社会解体の歯止めとしても作用していると見られる。
