先天性心疾患（Congenital Heart Disease：CHD）児の多くは，乳幼児期に外科的治療を行う。その為，入退院を繰り返しながら治療優先の日常生活を送っている。本研究の目的は，幼児期のCHD児と家族が家で取り組むことができる発達を促進する“運動と遊びのプログラム”を開発することである。3年計画で行う本研究の1年目である2022年度は，家における遊びの現状調査を行った。質問紙調査では，「お医者さんごっこ，おままごとなどのごっこ遊びや，絵本，ボール遊び，音の出るおもちゃ（太鼓，ピアノ，リコーダー，遊び唄など），ブロック，リズム遊び」などを実施していることが明らかになった。一方で，CHD児の母親の育児の様相の質的研究を実施した。その結果，母親は妊娠・出産・育児において普遍的なプロセスを経験していることが明らかになった。そのプロセスを辿りながら母親は，社会との交流の中で自律した育児をするように変化していた。文献レビューでは，重症CHD児の母体のストレスと胎児の脳の発達への影響としては，ストレス，不安，およびうつ症状のある母体のCHD胎児の小脳および海馬の発達障害がある。また，乳幼児期および学童期のCHD児は，脳の処理速度，数学的達成，注意および視覚空間認知機能，言語，記憶の認知領域，知能指数，微細な運動機能において健常児と比較して低下している。さらに，CHD児の社会的相互作用は，社会的認知，社会的コミュニケーションなどで低下していたが，社会的認識は問題ではなかった。幼児期は，“想像するちから”などのコミュニケーション力を獲得する時期であることを考慮して，次年度は，母親の経験している育児の様相に応じたCHD児の遊びの検討を行う。
