全国老人保健施設協会のホームページから、介護老人保健施設（以下、老健）の類型別割合に準じて200施設をサンプリング。各施設の管理者宛に依頼書と質問紙を郵送し、看護職と介護職を対象とした自記式質問紙による老健での看取りの現状と多職種連携に関する実態調査を実施した。看護職と介護職に各200部（計400部）を配布し、看護職55部・介護職52部を回収した（全体回収率26.8％）。看取り時の連携について、看護職・介護職ともに約8割が「連携が取れている」と認識していたものの、看取り時の課題は、「あり」が看護職61.8％、介護職66％と6割以上が課題を認識していた。課題の内容としては、看護職・介護職共に「情報共有不足」「他職種との連携不足」「職種間のモチベーションの違い」が多かった。職種ごとの特徴としては、看護職は「夜間医師不在時の負担」、介護職は「看取りについての知識不足」「看護師・医師のみで看取りを行っている疎外感」が挙げられた。看取りに対する不安について、「あり」が看護職41.8％、介護職50.9％であった。より良い看取りを実践していく上での方法としては「研修会の開催」「情報共有」「デスカンファレンスの実施」などが挙げられた。看護職・介護職以外でより連携を求めたい職種としては、MSWなどの相談職が最も多かった。老健での看取りは、概ね連携はとれていると認識しているものの、多職種で関わる故のモチベーションの違いや、介護職が阻害感を認識している実態が浮かび上がった。多職種での看取りを実践していくためには、職種を超えたケアに携わる全員での情報共有や、看取りを振り返る場の設定等の必要性が示唆された。
