我々は名古屋市における1歳6か月児健診時点の問診を中心としたスクリーニング基準と, 3歳児健診時点のう蝕経験状況との関係について検討し, 小児歯科保健事業の評価と今後の公衆歯科衛生的な事業の展開について模索して来た. 1981年度の研究結果から1歳6か月児健診時の重点的指導項目は(1)母乳の飲用, (2)哺乳瓶の使用, (3)間食摂取不規則, (4)間食回数1日3回以上, の4項目が3歳時のう蝕発生に最も関係していたことが認められた. このことから, 名古屋市では1982年度より上記の4点を重点的に指導することを各事業で実践してきた. 今回はこの指導を受けた小児とそれ以前の小児の歯科保健行動と3歳時点のう蝕経験状態の変化を比較することで事業の評価を行った. また1981年度と1985年度に行った1歳6か月児と3歳児のコホート分析調査から, スクリーニング項目の撰定について, 特異度, 敏感度, 相対危険度, 人口寄与危険度割合を用い検討した.その結果, 1981年時点に比べ, 歯科保健行動では重点的に指導を行った項目での明確な改善が認められた. ことに1歳6か月時点での離乳完了者の割合の増加は著るしかった. また1983年から1984年にかけ3歳時のう蝕経験者率が明確に減少したが, これは歯科保健行動の変化がかなり反映したものと考えられた. またこの保健行動の変化は, 1歳6か月健診時の保健行動によるスクリーニング基準の敏感度と特異度に影響したことが認められた. すなわちスクリーニング項目の陽性率の増加は敏感度を高め, 特異度を下げる傾向にあった. また重点指導項の相対危険度にはそれほどの変化は無く, 各項目のう蝕発生に対する危険度は一定のものであることが明らかとなったが, 人口寄与危険度割合で見た場合は各項目の減少は明確であった. 但し間食摂取不規則の項目は1985年時点でも高い値を示しており今後の対策が重要であると思われた.
