项目摘要

医療技術の高度化に伴う医療費用の増大、また、人口の高齢化による医療需要の増加など、医療を取り巻く条件は厳しく変化しつつある。このような時に、より良い医療を効率性をも配慮して国民に提供するには、医療機能を適確に評価することは重要な課題である。とくに、病院においては自らが保有する医療機能を明確にして、その地域的配置や医療施設相互間の機能分担と関連させて、患者へのサービスを客観的に評価することが、医療の質的向上において極めて重要である。しかしながら、わが国においては欧米諸国と異なり、医療の評価については、基準の設定など方法論についても、現実の実施の手法についても、殆ど確立していないのみならず、検討も消極的で、診療録管理に関連してのアプローチが主体であった。本研究においては、まず、欧米およびわが国において既に提示された機能評価に関する方式(JCAH,PIER,病院経営管理の基準等)から評価のためのチェック項目(評価項目)を整理、抽出した。次いで、これをわが国の病院の実情に合致するように手直しして、病院が自己評価すること、容易に実施できること、評価を行うことが改善に資することを原則として、評価項目の選定を行った。評価項目は、基本的事項、地域医療指向、患者サービスの向上、診療の学術性、管理運営の合理性の5つに分類した。設定した評価項目によって、宮城県下および全国の約300病院に病院機能評価調査を実施し、203病院から回答を得てこの集計結果から評価に有用と考えられる100項目を病院機能評価項目表として作成した。また、評価結果の数量化を試みるために評価項目の病院機能評価における重要度をデルファイ法で設定し、それぞれの重要度を基本に点数化の方式を定め評点による病院機能評価を実施できるようにした。また、評価項目についての解説を付した解説書を作成する予定であある。
