项目摘要

In chronic liver disease, the histopathological findings of the biopsied tissues, fibrosis of the portal area, inflammatory cell infiltration and piecemeal or focal hepatic cell necrosis are very important. However, they do not always explain the stage of disease. The degree of hepatic injury and fibrosis are changing from time to time on the balance of strength of the causative agents and regenerative capacity of the hepatic cells. In current study, we tried to detect S-phase hepatic cells in the biopsy specimen and compared with histopathological findings to estimate the prognosis.Material and Method: 20 cases of biopsied liver tissues were used for present study. 12 cases were chronic active hepatitis (CAH), 4 were cirrhosis (HC), 2 were chronic inactive hepatitis (CIH), 1 was chronic alcoholic injury (CAI) and 1 was healed acute hepatitis (AH). The liver tissues were incubated a hour in Eagle EME medium containing 3mg/ml Bromodeoxyuridine (BrdU), with 3 atmospheric pressure of 95% … More O_2 and 5% CO_2. After incubation, the tissues were fixed in cold ethanol and embedded in paraffin. Immunohistochemical staining by ABC method was performed using a monoclonal antibody against BrdU (MA-7580, Beckton-Dickinson). The BrdU positive hepatic cells were observed and the positive ratio (labeling index:LI) was calculated.Result and discussion: S-phase hepatic cells were demonstrated in all of the specimens examined, and LI were varied from case to case(1.8-27.3%). The cases of HC (22.4<plus-minus>4.2%), CAH (16.9<plus-minus>3.8%) and AH (27.3%) showed higher LI, and the cases of CIH (3.6<plus-minus>2.6%) and CAI (2.3%) showed low LI. Therefore, the hepatic cells of active chronic liver disease were revealed to regenerate rapidly. The degree of fibrosis was grade as non, slight, moderate, bridging fibrosis and septal formation, and compared with LI. The degree of inflammation and hepatic cell necrosis were graded as non, slight, moderate and marked. As the result, relatively well correlation was observed between LI and degree of fibrosis of the portal area.
