本年度は下記の3領域についての研究を進めた。(1)腫瘍マーカーについて:前立腺癌腫瘍マーカーとして、前立腺特異抗原(PA)が感度・特異性両面から最良であることは明らかにしたが、腫瘍マーカーとして臨床上最も意義のある根治手術を施行した症例が果して完治か否かを予知しうるかについて検討した。骨盤リンパ節廓清により組織学的に転移の有無を検索し、かつ3つのマーカー(PAP,PA,γ-Sm)とも検索したのは31例であったが、ここでもPAが最も良い指標となることが判った。即ち、リンパ節転移例(N_<(+)>)は10例すべて高値、N_<(-)>群では21例中10例が高値、11例が正常であった。このことから、PAが正常であればリンパ節転移はまずないと考えて良く、廓清術は省略でき予後良好であろうと考えられた。(2)前立腺癌モノクローナル抗体:前立腺肥大症組織を免疫原とし、BALB/Cマウスを感作し、脾細胞とマウスミエローマ細胞653と融合させ前立腺上皮細 胞と良く反応するクローンをスクリーニングした。このうち3種の抗体(MoAb)は、他臓器の上皮細胞と反応せず間接蛍光抗体染色により癌および正常前立腺上皮とのみ反応する膜抗原に対する抗体であることが判り、現在その特性と臨床応用の可能性につき検討中である。(3)現在までに500例以上の前立腺肥大組織を連続切片(幅3μm)にて組織検索し、23例の偶発癌を発見、それぞれについて細胞内DNA測光を施行、Gleasonスコアと対比しつつ新しい観点からの偶発癌の予後に関する指標を検討中である。なお手術後10年以上を経過し、消息の明らかな11例の偶発癌について、癌組織の全体に対する占拠度、Gleason sum score,分化度などをパラメーターとして予後、生存期間を検討したが、これらのパラメーターにおける一定の相関性は見出しえなかった。

(2)Adenocarcinoma antibody: Adenocarcinoma tissue immunogen, BALB/C, spleen cells, 653, fusion, Adenocarcinoma epithelial cells, good anti-tumor antibody These three kinds of antibodies (MoAb) are used to detect the epithelial cells of other organs, and indirect light antibody staining is used to detect the membrane antigens of cancer and normal prostate epithelium. (3)At present, more than 500 cases of prostate hypertrophy tissue sections (width 3μm) for tissue analysis, 23 cases of occasional cancer found, all of which were detected by intracellular DNA photometry, Gleason test results were compared with new ones, and indicators related to the future of occasional cancer were evaluated. More than 10 years after surgery, the information was clear, 11 cases of occasional cancer were detected, the proportion of all cancer tissues, Gleason sum score, differentiation degree, survival period, certain correlation was found
