健常者、半側空間無視(USN)及びその近縁症候である感覚消去現象を呈する症例につき、視覚性、触覚性課題に対する反応を検討した。1.動的視標の探索課題で、USN症例では連続的視標追跡に際しての滑動性眼球運動に比し、跳躍性視標追跡において衝動性眼球運動の障害がみられ、視覚対象の離散的な探索能力の低下が示唆された。2.触覚性線分二等分課題の遂行時に、探索指の運動軌跡と眼球運動(閉眼)を同時記録した。健常者では、殊に視覚心像依存性方略による場合いは探索指の運動と同周期の潜在性水平眼球運動を伴うが、位相差は被験者により一様でなかった。3.本課題で、USN軽症例では触覚性探索範囲は棒の両端に及び、ほぼ同期性の水平眼球運動がみられたが、二等分点は視覚性課題の場合と異なり左へ偏位する症例があった。重度な例では指の走査が左端に達しないことがあり、その際眼球の偏位も左方向には不充分であることが多く、二等分点は右に偏位する例もみられた。4.複雑刺激による触覚性消去現象検査(QET)の成積を健常者で標準化した。5.本検査を脳血管障害例に施行し、一側のいわゆる消去現象を示した症例のうち、消去側の触刺激の存在自体が知覚されず要素的触刺激においても消去現象を呈した群(A)と、複雑課題での素材の同定にのみ消去現象を示した群現象(B)とが見出された。各群の消去現象は夫々知覚及び認知レベルにおける刺激の競合により発現することが示唆された。B群の消去はその側への注意の促進により改善したが、A群では不変であった。A群は病初期にUSNを呈した症例を含み、病巣は感覚消去と対側の下頭頂小葉に共通していたが、B群では特定できなかった。6.脳梁損傷例につき、両側同時の要素的感覚刺激時に、挙手反応では正答可能で同時に口答反応では左側の刺激を無視する"見かけ上の消去現象"を呈する症例と、右手の動作及び言語性反応でのみ左側のUSNを呈する症例を新たに報告しその機序を検討した。
