上顎顔面一体模型作製は助調に進んでおり、初診時及び初回口唇裂手術直前までの印象を完了した症例は、総数約40症例である。その中の片側性唇顎口唇蓋裂20症例に対して、三次元計測器(三豊製作所マイクロコードMX203)を使用し、顔面頭蓋に対する上顎骨の三次元的成長発育を分析した。その結果、上顎の下方(口腔側)への発育は、歯槽頂付近よりも披裂縁付近の方が大きく、それにより上顎歯槽平面はV字形からより平担化の傾向が認められた。又上顎の前方への発育は、major segmentよりlesser segmentの方が大きく認められた。この分析結果は、第 回日本口蓋裂学会総会において講演発表した。今後も引き続き症例数を増やし裂型別による比較検討及び印象採得を初診時、口唇裂手術直前、口蓋裂手術直前、5〜6才時と施行し経時的な観察を行ないHotz型人工口蓋床の顎発育への効果に関して研究を進める予定である。

The model of upper jaw is used to adjust the impression before cleft lip surgery. The number of cases is about 40 cases. The three-dimensional growth and development of maxilla was analyzed by using the three-dimensional detector (MX203) in 20 cases of cleft lip and palate. As a result, the development of the lower jaw (oral side) is opposite, the top of the tooth groove is close to the side of the tooth groove, the side of the tooth groove is close to the side of the tooth groove, and the side of the tooth groove is close to the side. And the front of the upper jaw is growing, the major segment is growing, the lesser segment is growing, the major segment is growing, and the lesser segment is growing. The results of this analysis were presented at the Second Japan Open Society Conference. In the future, the number of cases of cleft lip will increase, and the comparative study and impression will be obtained at the time of initial diagnosis, before cleft lip surgery, before cleft lip surgery, at 5 ~ 6 hours, and at the time of implementation.
