项目摘要

部分膵自家移植の関する臨床研究:従来より、8症例(膵頭部癌7例、慢性膵炎1例)に対して、膵全摘・部分膵移植を行い、経口ブドウ糖負荷試験、経口脂肪(リボムル)負荷試験、経静脈的負荷試験(ブドウ糖0.5g/kg、アルギニン0.5g/kg、インスリン0.1U/kg)を、術前ならびに術後1ケ月目に施工していた。本年はさらに4例の膵頭部癌症例を加え、膵全摘・部分膵移植症例は合計12例となった。さらにこの12例中7例に対して術後6ケ月目に上記諸検査を施工しており、術後長期間にわたる移植膵の内分泌機能を検討した。その結果、部分膵自家移植により膵全摘後の膵内分泌機能の温存を計ることが可能であることが判明した。また、部分膵自家移植を行うことにより、膵全摘症例のQuality of lifeは著しく改善された。術後、膵癌により死亡した症例に対しては、可能な限り解剖を行い、組織学的にも検討を加えている。同種部分膵移植に関する研究:以上の臨床ならびに実験的研究成果を臨床に応用し、同種部分膵移植を計画中ではあるが、現在の日本国内における社会的理由から、実施に至っていない。
