歯科治療、特に麻酔・抜歯といった処置が患者におよぼすストレスについて、従来より研究してきたが、平成元年度科学研究費補助金にては呼吸気中の酸素量と炭酸ガス濃度を測定することを企画した。酸素消費量、炭素ガス排出量および肺換気量の測定には呼吸ガスモニタ-(RE-3000)を利用し、抜歯にあたり、局麻剤、血管収縮剤含有、【○!+】【○!-】に分け、術前より抜歯処理が終了した20分〜30分まで心拍数、血圧、ECGにてモニタ-記録したが、血圧と心拍数については、血圧連続監視装置BP-308ET(日本コ-リン社製)を用いた。心電図記録はECU-50B型心電計を時々使用した。測定時の体位は仰臥位または半仰臥位とし、呼気ガスはサンプル導入チュ-ブの先端を右あるいは左の鼻孔部にあて、患者の自由呼吸を行わせ、そのリアルタイムの記録をRE-3000にて行わせた。またダグラスバック方式を応用した例では、処置前、局麻を少し行った時期、局麻を終了した時点、抜歯を少し始めた時点、抜歯終了時点、すべての処置が終了して20分後について、バック内に集めた呼気がスサンプルの炭酸ガス排出量を検討した。本年度の研究実績記載の時期においては、まだ症例がわずかであるため何ら成果として述べられないのが、虚血性心疾患を有する患者、広義の意味での心肺機能を主とした予備能力の衰えた患者、糖尿病、甲状腺機能異常等を有する患者の歯科治療時の代謝の変化をも今後は臨床研究として進めたい。
