项目摘要

Fontan手術は複雑心奇形に対する機能的退治手術法として広く用いられているが、その手術適応には厳格な基準が設定されている。本研究は手術適応の拡大を目指し、Fontan循環の血行動態の特性の把握、循環補助システムの確立を目的として行なった。まず、イヌを用いてFontan手術モデルの確立を行なった。常温、心拍動下に、正常循環からFontan循環へ繰り返し移行可能なFontan手術モデルの作成に成功し、安定した状態においてFontan循環での血行動態の評価が可能となった。その結果、正常循環からFontan循環へ移行させると、平均動脈圧は約40%に、心拍出量は約30%に低下し、肺血管抵抗値は約2.4倍に上昇した。この状態に急速輸液により、10-15ml/kgの容量負荷を加えると、動脈圧は50%値以上に、心拍出量は45%にまで改善し、肺血管抵抗値は1.7倍値にまで低下し、Fontan手術後には十分な容量負荷が必要な事が確認された(第23回日本心臓血管外科学会総会において発表、1993.3.5.福岡)。次に右心補助システムによる右心機能補助効果についての検討を行なった。空気駆動型のcompressing deviceを作成して、Fontan手術モデルの右心房壁に装着し、右心房を外側より圧迫して右心機能補助を行い、動脈圧の25%の上昇、心拍出量の30%の増加をみた。更に、より安定性のある、心周期に同期させた右心房内バルーンパンピングによる補助循環法を開発した。その結果、動脈圧は正常循環の62%に、心拍出量は55%にまで回復させる事ができた。本研究の結果、Fontan手術における血行動態の特性は、体血圧の低下、心拍出量の減少、肺血管抵抗値の上昇である事が判明した。この血行動態の改善には十分な容量負荷と心房内バルーンパンピングが有用な手段であった。肺血管抵抗が上昇する機序の解明、臨床応用可能なバルーンのデザイン、駆動方法の開発が今後の検討すべき課題と思われた。
