労作性狭心症、一枝病変例に対して行われた経皮的冠動脈形成術(PTCA)成功例(残存狭窄50%以下の例)を対象としてPTCA施行前、3日後、10日後に症候限界性運動負荷試験を施行し、運動中の心電図をモニターした。さらに、運動負荷前後24時間の蓄尿を行い、プロスタサイクリン、トロンボキサンA_2の安定した代謝産物である11-dehydrothromboxane B_2および6-keto-prostaglandin F_1 alphaの濃度をRIA(PEG)法で測定した。そして、PTCA成功例において運動負荷試験による心電図上のST下降とアラキドン酸代謝の不均衡の関連およびこれらが再狭窄への進展の予測因子となりうるかどうかをPTCA施行3か月後に再狭窄の有無確認のため冠動脈造影を施行し検討した。この結果、再狭窄例においてPTCA施行前に行った運動負荷試験前後の尿中の11-dehydrothromboxane B_2濃度/6-keto-prostaglandin F_1 alpha濃度が運動負荷試験前に比べ運動負荷試験後に増加する傾向が認められた。しかし、再狭窄例のPTCA施行3日後、10日後の運動負荷試験では心電図上有意なST下降が認められたが運動負荷試験前後の尿中の11-dehydrothromboxane B_2濃度/6-keto-prostaglandin F_1 alpha濃度の変化は少なく、運動負荷試験における心電図上のST下降とアラキドン酸代謝の不均衡の関連は認められなかった。再狭窄の認められない例においてはPTCA施行前、3日後、10日後における運動負荷試験前後の尿中の11-dehydrothromboxane B_2濃度/6-keto-prostaglandin F_1 alpha濃度の変化は少なく運動負荷試験中の心電図上有意なST変化はなかった。以上より、PTCA後再狭窄へ進展する例ではPTCA施行前の運動負荷試験においてすでにアラキドン酸代謝の不均衡が認められ、再狭窄への進展の予測因子となりうる可能性が示唆された。
