有機酸代謝異常症の中のひとつであるメチルマロン酸尿症について検討を加えた。我々は以前にラットでメチルマロン酸尿症の病態生理モデルを作成した。すなわち、メチルマロン酸尿症の急性期では、メチルマロン酸よりもプロピオン酸のほうが血中、組織中の濃度が高いことに注目し、ラットにプロピオン酸を負荷することで急性期の状態を作成した。このとき、投与されたプロピオン酸は、まずCoAと縮合し、propionyl-CoAとなり、次にpropiony-CoA carboxylaseによりmethylmalonyl-CoAに変換されるが、この2つの反応はATPを必要とする。そこで、ATP製剤をプロピオン酸と同時に投与し、代謝動態に与える影響を、組織中有機酸濃度をGC/MSで測定することで検討した。ラットにプロビオン酸単独(10mmol/kg/hr)、あるいはプロピオン酸と同時にATP-MgCl_2(12.5mumol/2hr)を経静脈的に負荷した。プロピオン酸単独では肝臓のメチルマロン酸とプロピオン酸との比(メチルマロン酸/プロピオン酸比)は、約0.1-0.2となった。これをプロピオン酸と同時にATP-MgCL_2を負荷すると、その比は0.3-0.4と増加がみられ、プロピオン酸から、メチルマロン酸へのcarboxylationが、ATP-MgCL_2により促進されたものと考えられた。しかも、肝臓のプロピオン酸濃度の上昇も低かった。よって、我々の結果に基づけば、メチルマロン酸尿症の急性期においてATP-MgCL_2を投与することは、より有毒とされているプロピオン酸を、より尿中排泄のよいメチルマロン酸に変換することを促進し、結果的に各代謝の低下を防ぐことができると考えられる。今後、カルニチンを同時に負荷し、やはりATPを必要とするアシルカルニチンの生成が促進されるか、また、MRSで肝のATPが実際に増加するかなどについて検討する予定である。
