1.目的.非侵襲的で精度の高い骨塩定量法であるDEXA(dual energy X-ray absorptiometry)法を用いて骨塩減少の程度と頻度および骨代謝に関係する血液学的検索を行い、早期における骨障害発生予測の指標を検討した。2.対象.方法.対象は教室における胃手術後症例153例であった。術式は胃全摘55例、胃切除98例で、術後経過年数は3.3±4.0年であった。骨評価の方法としてはDEXA法を用いて全身骨および腰椎の骨塩量を求め、骨塩量低下の程度をscore化した。血液検査としては、血清Ca、P、BUN、Cr、Alp、Alp分画、intact PTH、BGP(Osteocarcine)を測定した。臨床症状は腰背部痛、こむらがえり様症状および骨折の有無をscore化した。3.成績.?.血清 Ca、P、BUN、Crは、術式・性別に関係なく正常であった。intact PTHは29.4%が異常値であった。PTHを正常群、異常群に分けDEXA scoreの平均を求めた。術後1年未満の症例では正常群0.55±1.00、異常群1.35±1.81で有意差はなかったが、1年以降では、正常群0.63±1.36、異常群3.26±1.99と有意差が認められた(P=0.01)。A.BGPは17.6%が異常値であった。BGPを正常群、異常群に分けDEXA scoreの平均を求めた。術後1年未満の症例では正常群0.64±1.19、異常群1.64±1.91で有意差が認められ、1年以降の症例でも、正常群0.98±1.60、異常群3.94±2.08と有意差が認められた(P=0.01)。4.まとめ.BGPおよびPTHは骨塩低下のよい指標であり、骨障害発生を予測する指標となり得る可能性が示唆され、今後も症例数をふやして検討を続ける必要性が明らかとなった。
