抗ラミニン抗体を用いて胃癌の切除切片を染色し、神経の染色性をS‐100染色、鍍銀染色と比較検討したところ、ラミニン染色ではnerve fiber表面と、perineuriumが明瞭に染色されることを明らかとした。このラミニン染色を用いて胃癌の神経浸潤(PNI)を判定し、PNIと病理組織学的所見との関係、各種進展形式との関係、再発形式との関係について検討した。胃癌切除283例の検討を行ったところ、PNI陽性の癌病巣は、肉眼型は浸潤型、癌占居部位は全体型、周在性は全周性に多く、組織型は未分化型、浸潤増殖様式はINFgammaに高率にみられた。進展形式および再発形式との関係では、腹膜播種転移および腹膜再発とPNIは関係があり、特に壁深達度T2おける腹膜再発と有意の関係がみられた。Coxのハザードモデルによる多変量解析を行ったところ、PNIがもっとも予後に関与する因子であるという結果が得られた。以上の結果を近畿大学医学会誌に掲載し、研究分担者の学位論文と認められた。今後さらにT2胃癌症例を増やし、PNIと腹膜播種転移、腹膜再発との関係を検討していく予定である。また、以前から施行している術中洗浄細胞診とPNIとの関係についても検討する予定である。

Anti-tumor antibody was used to stain gastric cancer resection sections, nerve staining, S-100 staining, silver staining, comparative staining, nerve fiber surface staining, perineurium staining, and so on. This paper discusses the relationship between PNI and histopathological findings, the relationship between various forms of progression, and the relationship between forms of recurrence. 283 cases of gastric cancer were resected, and PNI positive cancer disease was reversed, macroscopic type was reverse invasive type, cancer occupied site was reverse circumferential type, histological type was reverse differentiated type, invasive type was reverse INFgamma high rate. The relationship between the form of progression and the form of regeneration is different from that between the form of peritoneal seeding and the form of peritoneal regeneration. The results of Cox's analysis are as follows: The above results are reported in Journal of Medical Association of Kinki University and the dissertation of the researcher is acknowledged. In the future, the number of T2 gastric cancer cases will increase, and the relationship between PNI and peritoneal metastasis will be discussed. The relationship between the previous operation and the PNI was determined.
