心内膜床欠損症の左室流出路形態について病理標本40例を用いて病型別に検討した。その結果は以下の如くであった。(1)左室流出路周径の上行大動脈周径に対する比は病型別に有意差がなく平均値は約1.0であった。完全型(Rastelli分類A型)の1例のみが0.65と著明な左室流出路狭窄を示した。(2)心室中隔scoopingの程度は完全型(A型)と中間型で強く、部分型と完全型(C型)は軽度であった。(3)anterolateral muscle bundle(ALMB)が左室流出路に占める割合は中間型で最も大きく、部分型、完全型(C型)で有意に小さかった。(4)ALMBが著明に拡大すると、ALMB自体が大動脈弁に対して右方(無冠尖方向)に進展するため、その結果左側房室弁(superior bridging leaflet)と大動脈弁間の線維性連絡部が左室流出路径に占める範囲は狭小化した。(5)ALMBが著明に進展すると左室流出路は扁平化し、将来的な左室流出路狭窄を起こす要因の1つになりうると推測された。(6)著明な左室流出路狭窄を示した1例では著明なALMBの突出と肥大、superior bridging leafletの腱索の付着異常が左室流出路狭窄の主な原因であったが、後者はさらに房室弁の運動制限を来たし、弁逆流にも大きく関与していたことが推測された。(7)以上より、心室中隔のscoopingによる左室流出路の潜在的狭小化を解剖学的特徴とする心内膜床欠損症の左室流出路形態は病型により差が見られ、scooping以外に特にALMBの形態が左室流出路狭窄に関与する重要な因子と思われた。大血管転位症の左室流出路形態を現在研究中であるが、心内膜床欠損症と異なり、ALMBの意義は少なく、ventriculoinfundibular foldの左室流出路形態におよぼす影響が大きいようであるが、今後さらに研究を進めて明らかにしていきたい。

In 40 patients with endocardial bed deficiency and left ventricular outflow pathway, 40 patients were treated with different disease types. The following results are as follows. The main results were as follows: (1) the circumference of left ventricular flow was significantly lower than that of the disease type (about 1.0%). Complete type (Rastelli classification A) 1 case showed left ventricular outflow stenosis. (2) the degree of scooping in ventricular septum was complete (type A), strong in middle type (type A) and severe in partial type (type C). (3) anterolateral muscle bundle (ALMB) "left ventricular flow outlet" accounted for the largest type, partial type and complete type (type C). (4) ALMB shows that the left ventricular outflow pathway in the left atrioventricular (superior bridging leaflet) segment of the left atrioventricular mass (superior bridging leaflet) has narrowed down the range of the left ventricular outflow path. (5) ALMB indicates that the progression of left ventricular flow outlet flattening, and the future stenosis of left ventricular flow outlet is due to the increase of left ventricular flow outlet stenosis. (6) the diagnosis of left ventricular flow outlet stenosis was demonstrated in 1 case. ALMB highlighted hypertrophy, superior bridging leaflet chordae tendineae were responsible for the main cause of left ventricular flow stenosis, and the latter showed that the atrioventricular movement limit was limited, the countercurrent was affected, and the hypertension was induced. (7) the potential narrowing of the left ventricular outflow outlet in the ventricular septum (scooping), the anatomical characteristics of endocardial bed deficiency, the left ventricular outflow pathway, the disease type, the special ALMB pattern outside the scooping, the left ventricular outflow outlet stenosis and the important factor. The pattern of left ventricular outflow in patients with macrovascular disorder has been studied in recent years, including endocardial bed insufficiency, endocardial bed deficiency, ALMB, ventriculoinfundibular fold, and so on.
