项目摘要

粘膜負担様式を含む義歯症例における床の適合性の変化には、顎堤の吸収と、粘膜の塑性変形とが含まれると考えられるが、粘膜の塑性変形による床の適合性の変化は、比較的短期間で生じる可能性が高い。そこで、本研究においては、装着直後より1週間にわたって義歯の咬合接触強さおよび床の適合性の変化について検討を行なった。臼歯部に遊離端欠如を有する7症例を被験対象とし、それぞれにクラスプを支台装置とした実験用遊離端義歯を作製した。これらの実験用義歯の床基底面の条件は、粘膜静止印象と加圧印象との2種類である。いずれの義歯においても、その装着時の咬合接触強さを、咬合紙の引き抜き法によって確認した。義歯装着直後、1日後、3日後および7日後の各観察日において、咬合紙の引き抜き法による義歯床の沈下量および義歯床の適合性の検査を行なった。結果の概要は以下のとうりである。1.加圧印象による義歯については、義歯装着時に咬合紙1枚の接触強さに調整を行ったが、装着後7日までの範囲においては、沈下量の増大は認められず、床の適合性の変化も認められなかった。粘膜静止印象による義歯については、義歯装着時には咬合紙3枚〜10枚の接触強さを示し、装着後7日までの義歯床の沈下量の増大は1枚〜10枚に分布し、床の適合性の変化が生じるものが約半数存在した。以上の結果から、加圧印象による義歯床に対して、装着時に咬合接触強さの管理を行なうならば、装着直後の粘膜の塑性変形に伴う適合性の変化はほとんど生じないことが指摘される。

The above results show that the pressure impression is significant, the occlusal contact strength is managed during installation, and the plastic shape of the mucosa is adjusted after installation.
