项目摘要

地域および訪問看護を行っている病院のエキスパート保健婦(ここでは、5年以上の保健婦の経験を持ち、ほかの保健婦が専門的アドバイスを求めるような人と定義する)に面接による聞き取り調査を実施した。計画段階では病院保健婦は対象に入れてなかったが、わが国における近年の地域保健医療福祉の推進により、病院保健婦の働きも重要となってきているため実施段階で取り上げた。本年度受け行け可能であった施設は、新潟県新発田市健康管理センター、長野県川上村役場、山梨県河口湖町日本赤十字病院、山梨県山梨市厚生病院の保健婦、計22名であった。聞き取り調査の内容は、地区診断の定義、地区診断の実施状況、地区診断評価の時期と方法、地区診断を行うにあたり必要な情報、地区診断の研修、地区診断と予算について、地区診断と保健婦活動の評価について、地区診断実施上の困難点についてである。また、各施設毎にグループ・ディスカッションを行い、現在のわが国における保健婦活動の中に占める地区診断の現状と課題、現場の保健婦のニーズについて、ブレイン・ストーミングを実施し、今後の研究課題の明確化をはかった。地区診断の困難点で最も多かったのは、地域の問題の優先順位の付け方であった。そこで、米国の看護学研究者Dr.Mueckeの開発した7つの評価項目からなる優先順位決定のためのフォーマットを提示し、各保健婦にひ実際にこのフォーマットによる地区診断を実施してもらった。エキスパート保健婦であったため、基本的な地区の情報の整理がされており、平均2時間の作業でDr.Mueckeのフォーマットによる地区診断ができた。22名中20名が、この作業を通して地区の問題が明確になったと評価した。わが国の保健婦は、地区の基本的な情報収集・整理および事業のまとめを年度単位で毎年行っているところが多いので、「地区診断フォーマット」の導入の可能性と有効性が示唆された。昨年7月に米国でDr.Mueckeとミ-テイングを行い、わが国における地区診断ツールの開発の研究への協力を継続的に得ている。保健婦の持っている地区の基本的な情報と固有な情報を地域の看護診断にむすびつけられるよう、来年度は引き続き数量的な分析を行う予定である。
