成人では自律神経機能の診断法として、心電図R-R間隔のパワースペクトル解析が用いられ、種々の病態での変化が報告されている。胎児仮死の診断法の1つとして、このパワースペクトル解析の有用性について検討した。胎齢125-135日(満期145日)の妊娠羊を用いて羊胎仔慢性実験モデル作成し、自律神経遮断実験および胎児仮死実験を行い、連続する200心拍について自己回帰モデルを用いた線形予測法を用いたR-R間隔のパワースペクトル解析を行った。胎仔にpropranololを投与することにより、0.05-0.15Hzの低周波領域のピークはほぼ消失したが、atropine投与では同領域が減少するもののピークは残存した。この結果は成人における報告と同様であり、胎児においても同領域が副交感神経系に修飾された交感神経活動を表していると考えられた。次に胎児仮死におけるスペクトルパターンの変化を観察するために、母体への窒素投与および臍帯圧迫による実験的仮死モデルを作成した。Initial hypoxemiaにより0.05-0.15Hzの低周波領域が有意に増大し、acidemiaにいたるとパワーのフラット化がみられ、特に0.05-0.15Hzの低周波領域が低下する傾向がみられた。このことはストレスに対する胎児交感神経系の代償的興奮および引き続く機能不全を表していると考えられた。以上より胎児心電図R-R間隔のスペクトル解析は、胎児仮死の診断に有用である可能性が示唆された。今後は、その他の周波数領域や未熟胎仔における検討が必要であると思われた。
