目的:心因性視機能障害は、小児に多くみられ、しばしば視野に高度の狭窄がみられても日常生活にそれほどの支障がないことが多い。今回我々は、このような症例においてスクリーン上に図形や絵をみせた場合にどのような反応をし、注視点はどのような動きを示すかについて検討を行った。対象および方法:対象は心因性視機能障害の児童、生徒22名(男性6名、女性16名)で、平均年齢は11.8歳である。対照群は28名(男性10名、女性18名)で平均年齢は12.2歳である。さらに患者群は、視野狭窄の程度により20度未満を高度異常群(4名)、20度以上40度未満を中程度異常群(10名)、40度以上60度未満を軽度異常群(8名)とに分けて検討を行った。評価した項目は、注視点の数、注視点のばらつき、注視点の総移動距離、注視点の平均停留時間、注視点間の平均移動速度、想起得点の6つである。結果および考察:総移動距離の短縮、平均移動速度の低下、平均停留時間の延長傾向がみられたが、S字と公園との間で明かな傾向の違いみられなかった。注視点のばらつきは、公園で有意な低下がみられた。これは日常生活場面に近い状況では見落としが多い傾向を示し、外界に対する興味の低下が示唆された。想起については、患者群で有意に低く、特にS字よりも公園のようなより複雑な図版においてその傾向が強くみられた。これには積極的な準備状態の低下も関与しているのではないかと思われた。今後、このような症例を長期にわたり経過を追い注視点や想起についてどのように変化してゆくか検討する予定である。
