歯髄血流減少時におけるレーザー切削と高速切削による窩洞形成後の歯髄反応を観察するために以下の実験を行った.イヌを用い,四つの実験群に分け,窩洞形成を行った.すなわち,下顎は局所麻酔を行った後で窩洞形成する局所麻酔(+)群とし,上顎は局所麻酔をせずに窩洞形成する局所麻酔(-)群とした.さらに,上下顎それぞれの左側あるいは右側にエアータービンによる切削を行ったものを高速切削群とし,その対側にレーザー切削を行ったものをレーザー切削群とした.窩洞はセメント仮封を行い,4日後に被験動物を安楽死させ,歯髄反応を病理組織学的に検索した.高速切削群において局所麻酔(+)群と局所麻酔(-)群とを較べたとき,両群ともに歯髄の出血が観察されたものが多かったが,局所麻酔(-)群ではより広い範囲に認められる傾向があった.また,局所麻酔(-)群では象牙細管内に棹状体が出現したが,局所麻酔(+)群では出現しなかった.一方,レーザー切削群における局所麻酔(+)群は,局所麻酔(-)群よりも歯髄の出血および象牙芽細胞の配列の乱れ・消失がやや広範囲に認められた.しかし,炎症性細胞浸潤や歯髄組織の変性の程度には,ほとんど違いがなく,両群ともに象牙細管内に棹状体の出現が観察された.局所麻酔(+)群におけるレーザー切削群と高速切削群とを比較すると,歯髄の病理組織学的所見にほとんど差がなかった.歯髄血流量の低下した状態でレーザー切削が歯髄に及ぼす影響は,高速切削と同程度であると思われた.
